Outpatient Mental Health Therapist
Job Summary

The Outpatient MH Therapist is a part of the multidisciplinary team responsible for providing direct clinical services to clients and their families by diagnosing and treating mental and emotional disorders, whether cognitive, affective, or behavioral. Completes Psychosocial Assessments; providing individual, group and family therapy for clients; facilitating psycho-educational training, family meetings, discharge planning; assisting in program development; developing, implementing, and reviewing Treatment Plans; and providing linkage to outside resources.

Job Duties
  • Conducts a psychosocial assessment and develops an initial discharge plan.
  • Develops and implements treatment plans for clients.
  • Conducts individual, group or family sessions.
  • Applies psychotherapeutic techniques and interventions in the delivery of services to individuals and families for the purpose of treating such diagnosed emotional and behavioral disorders.
  • Participates in multi-disciplinary treatment planning conferences. Presents appropriate client information to the treatment team. Recommends effective treatment interventions.
  • Maintains a caseload as assigned.
  • Timely prepares progress notes and treatment updates in the medical record. Maintains all clinical documentation in accordance with regulatory and accrediting standards.
  • Ensures each assigned client has a structured and clinically appropriate post care plan. Acts as Liaison to referral sources and support of continuation of care.
  • May coordinate various clinical and psychosocial or educational activities with clients and their families.
  • Evaluates client’s need and determines if referrals to other programs or facilities are needed.
  • May assist in the training of new clinical staff and interns.

Required Qualifications
  • Must have a Master’s in Counseling, Social Work, or Marriage and Family.
  • Must have LPC/LPCC, LSW/LISW or MFT/IMFT in the State of Ohio.
  • Must have a valid Ohio driver’s license.
  • Must have reliable transportation.
  • Strong verbal and written communication skills required.
School-Based Therapist
Job Summary

Under supervisory direction, assesses, diagnoses, counsels, and provides psychotherapeutic treatment, rehabilitation, and care coordination for behaviorally and emotionally disturbed children and adolescents as it is medically necessary; provides educational and consultation services to other staff, school staff; and performs related duties as required.

Job Duties
  • Interviews potential clients to assess the need for direct mental health treatment of mental and emotional disorders. Provides comprehensive medically necessary and psychotherapeutic services for individuals. Assist individuals in achieving productive social adjustments and help them to achieve greater living, vocational, recreational, and interpersonal goals through the modalities of individual, family and group counseling.
  • Diagnostically assesses, formulates and completes the client’s Treatment Plan inclusive of diagnosis, goals, and appropriate signatures within 30 days or the first 2 sessions from initial client contact, whichever is shorter.
  • Shares and receives appropriate information with the teachers, guidance counselors and administrative staff to assist in a collaborative effort with clients in the assigned buildings. Release of information forms must be signed by the parent / guardian prior to exchanging confidential information.
  • Educates clients and their families regarding their diagnosis, prognosis, treatment, and therapeutic process.
  • Works cooperatively with teachers, guidance counselors, and administrative staff within their assigned buildings including, but not limited to, relaying appropriate information to and from school facilities, assessing needs of additional staff, and coordinating services within the building.

Required Qualifications
  • Minimum Master’s Degree from an accredited College or University in the field of Social Work, Marriage and Family Therapy or Counseling.
  • Licensed in the State of Ohio to provide mental health services (LPCC/LPC, LSW/LISW, MFT/IMFT).
  • Demonstrated experience in the treatment of children, adolescents, adults and families individually and in group modality.
  • Ability to problem solve complex mental health problems including some crisis resolution.
  • Ability to develop and write clear, concise documents involving client outcome reporting requirements.
  • Effective time management and interviewing skills.
